Southampton surgery goes digital

Digital Health Specialist Nurse Alison Blythin on how a community practice has embraced my mhealth digital healthcare apps.

At the heart of a Southampton community, Townhill Surgery has transformed its healthcare delivery by offering patients access to digital health. It is the first practice in Southampton to implement into its clinical service, all 4 my mhealth long-term condition apps for patients with COPD, Asthma, Heart Disease and Type 1/2 Diabetes.

The my mhealth nursing and digital transformation team worked with the surgery to set up the clinical teams to assist patients with self-management, rehabilitation and the remote monitoring and management of their patients. To raise awareness, the surgery team proactively contacted their patients and hosted a my mhealth page on their website to help signpost patients to more detailed information before they registered an interest.

The implementation started just prior to Covid-19, so couldn’t have come at a better time for both the surgery healthcare team and patients. In March 2020, the first patient was registered. In just 2 months the practice has successfully onboarded 128 patients and over 70% are regularly using their apps to manage their condition.

Patients are able to input their health data as often as they wish to self-manage their health. This information is clinically monitored through the sophisticated clinical dashboard by their healthcare team without having to visit the surgery unless absolutely necessary. Clinicians are able to monitor symptom scores and health trajectories to determine a decline or improvement in health status. Additionally, health assessments and annual reviews have been carried out remotely with patients able to complete their CAT/ACT scores, monitor correct inhaler technique and take part in pulmonary rehabilitation.
